Surgical Interventions
1. Le-Fort maxillary advancement
2. Saggital split mandibular advancement
Various surgical interventions have been developed to treat OSA, including
- uvulopalatopharyngoplasty (UPPP)
- maxillomandibular advancement (MMA)
- tongue base reduction
The choice of surgical intervention depends on the patient's individual anatomy and the severity of their sleep apnoea.
UPPP involves the removal or repositioning of excess tissue in the throat, including the uvula, soft palate, and pharynx, to widen the airway and reduce the occurrence of obstructive sleep apnoea episodes.
MMA involves surgically repositioning both the upper and lower jaws forward to increase the size of the airway, to reduce obstructions.
Tongue base reduction involves surgically removing a portion of the tongue's base. The remaining tissue is sutured together, creating a smaller, less obstructive tongue that is less likely to collapse and obstruct the airway during sleep.

Surgical Success Rates
Surgical success rates vary widely, with MMA demonstrating the highest success rates in appropriately selected patients.
However, surgery carries inherent risks and complications, and the decision to pursue surgical intervention should be carefully considered.
